PaediCRID is the West Midlands regional reporting system for reporting incidence and excellence, where one or more West Midlands Hospital is involved in the care of a patient.  

The PaediCRID system was developed in 2017. Since its launch, PaediCRID has led to the identification of incidents and opportunities for education, safety and quality improvement.  

In 2019 the option of submitting examples of excellent practice was added to the system, this enables teams to share good practice and identify where excellent practice in one site may be replicated in other sites to improve patient care and staff experience.   

Whilst each network may have an individual governance process, there is value in developing an overarching multi-network West Midlands overview, as many specialties have interdependencies that will cross more than one individual network. 

The use of the PaediCRID system differs from submitting internal DATIX/reporting systems in that it allows a process for formalising anecdotal discussions and evidence and developing the provision of data. It gives space for concerns to be raised that may not be appropriate through an internal reporting system.

Exception Reporting

Exception reporting is a  well established process within neonatal networks, and provides a mechanism to highlight and identify challenges with care pathways, that may not fall into the PaediCRID reporting process.

Exception reporting serves multiple purposes including; Monitoring & Improvement, Resource Allocation & Data Collection

Anyone involved in patient care can complete an exception report.

The aim of the exception reporting process is collect data to support concerns we have been made aware of in the networks including, but not exclusively:

  • Delayed repatriations
  • Delays in patients being accepted into tertiary care
  • Care being delivered in non commissioned areas, for example HDU care in non HDU areas

The WMCN encourage the reporting of these exceptions to enable us to support teams with highlighting these challenges with our regional commissioning teams.

Exception reports will be collated and shared with trusts on a quarterly basis.
